Once you have booked with us, we will set up an appointment with you to open your file.  

The following forms will be handed out to you in order to help you plan your wedding.  Please check the return dates to ensure that it is available to us on time.

 
FORM TO BE FILLED IN BY RETURN DATE
1. Terms & Conditions To be signed by person responsible for payment /or bride ASAP - fax back to 011 252 6147
2. Timetable for wedding To be filled in by couple, one copy to Master of Ceremonies, one copy back to Mellow Oaks Day of final payment - 7 days prior to function
3. Table seating plan If we are not printing it, a duplicate copy must be handed in On day of rehearsal
4. Roadmap Please include a copy in each invitation Make own copies, or to be given to printers
5. Winelist To be filled in by person paying for wine 7 Days prior to function
